What is a Caregiver and what do they do?

Can you help us figure out what to do after a hospital or rehab stay?

Yes. As a Licensed Social Worker, we help families navigate hospital and rehabilitation systems, walk you through the community resources available, and build a plan for a parent who needs more assistance at home. We also provide specialized post hospitalization and post rehabilitation care once they're back.

What are some examples of high fall risk indicators with the elderly?

Unsteady walking, difficulty rising from a chair, poor vision, medication side effects, and clutter or poor lighting at home all raise fall risk. We assess these during the consultation and adjust the care plan around them.

What is the impact of having a caregiver?

A caregiver brings consistent daily support, safer routines, and peace of mind for the family — allowing your loved one to remain at home rather than moving into a facility.

Besides caring for the elderly, what else can a Caregiver provide?
